20F Conference Center Covid Guidelines

The American College of Surgeons adopted COVID-19 Guidelines to reasonably ensure the safety and security of our workplaces as a result of the Coronavirus Disease 2019 (“COVID-19”).
As a visitor, guest or vendor coming into our workplace or attending an ACS-sponsored event, the College requires that you follow the Mandatory Vaccination Policy we have adopted. Some important protocols and requirements that you must understand prior to your visit to the Chicago/DC office or meeting venue are:

a. Self-Check - Prior to your arrival at the College, you agree to conduct a self-check for COVID-19 symptoms. If you feel ill, for any reason, do not come to ACS offices. The CDC offers a self-checker here: https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/coronavirus-self-checker.html

b. Mandatory Vaccine – You agree to either being fully vaccinated (two weeks past your second dose in a 2-dose series, such as Pfizer or Moderna vaccines or 2 weeks after a single-dose vaccine, such as Johnson & Johnson’s Janssen vaccine) or show proof of a negative test (not self-administered) completed within 24 hours of participation in that day’s meeting.

c. Proof of Vaccine - Upon your arrival, you agree to show proof of your vaccine status to a representative of ACS (Receptionist or Meeting Coordinator) via either using CLEAR mobile app or proof of vaccination card with an ID. ACS will not retain any vaccine records. You should not send or email your vaccination status to ACS. If unvaccinated a daily negative COVID test, taken within 24hrs prior to attendance in each day of a meeting, will need to be provided prior to entering meeting (self-administered/home tests are not acceptable, please reach out to your meeting host for a list of approved locations near the meeting).  If available, we will be happy to offer alternative arrangements for virtual participation.

d. Face Coverings -  Masks are optional for all vaccinated guests. If unvaccinated, you agree and must wear a mask throughout the meeting.  If meeting is held at the ACS office (Chicago or DC), and guest is unvaccinated, mask must be worn throughout the building  and in common areas such as the lobby, hallways, bathrooms.

e. Physical Distancing - You agree to minimize face-to-face contact, avoid “close contact,” and maintain appropriate physical distancing between you and others.

f. Hold Harmless Waiver -  (Registrants will not be able to register for or attend in-person if not consenting) You agree  that on your behalf and/or on behalf of those registering-- I acknowledge that travel to and attending in person meetings entails the risk of contracting communicable diseases; I agree to waive and hold the American College of Surgeons, the facility, and each of their agents harmless from and against any liability, damages or expenses arising from travel to or attendance at the event that result in contracting communicable diseases; I agree to follow all health and safety protocols announced by the American College of Surgeons, the facility, and governmental authorities. I confirm that I am authorized to make these agreements for each of the individuals I am registering/attending this meeting.

**Please complete the information below either upload your proof of vaccine into the CLEAR app or provide your ID and vaccination card upon entrance (for a visual review) to the ACS representative once you are onsite.
